Application packets should include the following:

*Before mailing your completed packet be sure you have filled out the Applicant Intent Form found here. Do not fill out the form until you are ready to submit a completed application packer for the upcoming semester deadline.

      • VDOE Career Switcher application, with original signature (pp. 4-5)

      • Report on Experience form(s) signed by HR representative (p. 6)

      • Criminal History Waiver form (p. 7)

      • PRAXIS Subject Assessment Official Score Report 

      • VCLA Official Score Report

      • Official transcripts from all college/graduate-level coursework 

      • Active Adult CPR/AED/Basic First Aid card, hands-on training component is required

      • Dated Certificate of Completion for DSS Child Abuse and Neglect Module 

      • Dated Certificate of Completion for VDOE Dyslexia Awareness Module 

      • Dated Certificate of Completion for Restraint and Seclusion Module
        

      • Dated Certificate of Completion for Cultural Competency Module
      • Professional Essay  (maximum 750 words).  See “Process ” page for essay prompt.
